Discharge Planning

Discharge Planning is evaluated on the day of admission and onward. It is also discussed by the entire team before the 21st day of the Care Plan meeting with resident and family members. The Social Worker discusses options with resident/family members regarding rehab potential, entitlements, desired length of stay, and type of services provided in the community. When resident/family members request for discharge, the social worker makes referrals to appropriate home care agencies including Isabella's Long Term Home Health Program.

S.O.S. (Save Our Seniors) Program

The S.O.S. Program provides residents, who are being discharged, with free cellphones (battery and charger included). There is no airtime included, however the phone can make 911 calls at no charge. Please consult the Discharge Planner for further information.
